  1. My Plan: 24/7 access to a user-friendly platform for patients to create, manage, and share their advance care plans, ensuring their healthcare wishes are known and respected.
    1. An Advance Directive (AD) is a vital legal document that allows you to outline your medical care preferences in case you become unable to make decisions for yourself. It includes two main parts:
      1. Living Will: Details your wishes regarding medical treatments you want or do not want, such as life support or resuscitation.
      2. Medical Power of Attorney: Appoints a trusted person to make healthcare decisions on your behalf if you are incapacitated.
  1. Stay Updated: Review and update your directives as needed to reflect any changes in your health or preferences. Life is dynamic, and your healthcare plan should be too.

Basic Plan – $30 for a one-year subscription

With the Basic Plan, patients receive:

  1. Notarized Advance Directive for 1 Year
    Patients can complete and notarize their Advance Directive, ensuring their preferences are legally recognized.

    1. Share your ACP documents with your medical decision-maker, loved ones, or healthcare providers.
    2. Recipients receive a secure invitation via text or email to view the documents.
    3. Convenient sharing options allow for automatic transfer of ACP documents to healthcare providers’ electronic medical records (EMR), with easy download/upload options.

Premium Plan – $50 or a one-year subscription

The Premium Plan includes everything in the Basic Plan, plus:

  1. Automated Reminders
    Receive automated reminders for document reviews and alerts for serious health events.
  2. Video Messages:
    Recording a video message adds a personal touch, making your wishes clear and undeniable. It’s a powerful way to communicate your preferences directly to your loved ones and care team.
