By using the Website, you affirm that you are a licensed healthcare professional (including, but not limited to, doctors, advance practice providers, nurses, clinical social workers, chaplains, and other licensed healthcare professionals) and that you accept and agree to comply with these Terms. If you do not meet these qualifications or do not agree to these Terms, you must immediately cease using the Website. 

2. Authorized Use

2.1. Healthcare Documents: The Website provides healthcare professionals with the tools and resources necessary to retrieve, complete, and manage healthcare documents on behalf of patients. You may only use the Website for lawful and authorized purposes, including the retrieval and completion of healthcare documents with and for your patients.

2.2. Patient Information: You may only search for and access patient information on the Website if you have proper authorization to do so. Unauthorized access to patient information is strictly prohibited and will be subject to prosecution to the fullest extent of the law.

3.1. You may be required to create a user account to access certain website features. You agree to provide accurate, current, and complete information during the registration process and to update such information to keep it accurate, current, and complete.

5.1 If Subscriber provides any feedback to Empower Hope concerning the functionality and performance of the Services ( including identifying potential errors and improvements), Subscriber hereby assigns to Empower Hope all right, title, and interest in and to the feedback, and Empower Hope is free to use the feedback without payment or restriction.

6. Prohibited Activities

6.1. You agree not to engage in any of the following prohibited activities while using the Website:

• In any way that violates any federal, state, or local law or regulation or professional rule applicable to you and your use of the Services, including, without limitation, those relating to patient privacy, medical care and treatment, including the Health Insurance Portability and Accountability Act (HIPAA), physician self-referrals (the Stark law), or text messaging (the Telephone Consumer Protection Act).
• In any way that violates or conflicts with any agreement to which you are a party, including any agreement with your employer.
• Unauthorized access to patient information.
• Use of the Website for any unlawful purpose or in violation of applicable laws and regulations.
• Posting or transmitting any content that is harmful, threatening, abusive, harassing, defamatory, vulgar, obscene, or otherwise objectionable.
• Attempting to gain unauthorized access to the Website or its systems.
• Interfering with the proper functioning of the Website.
• Impersonating another person or entity or providing false information.
• To engage in any other conduct that inhibits anyone’s use or enjoyment of the Service or which, as determined by us, may harm Empower Hope or users of the Service.

• Introduce into the Service any viruses, Trojan horses, worms, logic bombs, or other material that is malicious or technologically harmful.
